When the donor comes back for patch removal, retrieve the relevant chain of custody form from your records.
Firstly, match the I.D. number on the chain of custody form with the number on the outside of the patch. If they correspond, note the removal date of the Sweat Patch on the form. Both the observer and the donor should verify this information and initial the top right section of the form designated for removal.
After the absorbent pad is placed in the specimen bag and sealed with the security label (refer to the section titled ‘PharmChek® Sweat Patch Barcode Labels’), the donor should sign the chain of custody form.
Should the numbers not match, this might indicate that the client has replaced their original PharmChek® Sweat Patch with one from another source, possibly to conceal drug use. It is highly recommended that agencies keep all unused PharmChek® Sweat Patches securely locked to prevent donors from accessing them.
